Indications: Ondansetron HCl 4 mg is indicated for the prevention of nausea and vomiting associated with emetogenic chemotherapy, including high dose cisplatin, and radiotherapy. It is also used in the prevention and treatment of post-operative nausea and vomiting. The Ondansetron HCl 4 mg medication above is manufactured by Sun Pharma. Do not use Ondansetron HCl 4 mg if you are allergic to ondansetron or any of its ingredients. Avoid using it if you are taking apomorphine. It is because Ondansetron HCl 4 mg may cause severe low blood pressure and loss of consciousness.
Ingredients: Ondansetron HCl 4 mg contains the active ingredient ondansetron hydrochloride. Some inactive ingredients are anhydrous lactose, microcrystalline cellulose, pregelatinized starch (maize), magnesium stearate, triacetin, titanium dioxide, and hypromellose.
Directions: Follow your doctor’s prescription exactly. You can take Ondansetron HCl 4 mg in tablet, orally disintegrating tablet, or oral solution form. The doses are the same for each form.
- For adults receiving highly emetogenic chemotherapy, take a single 24 mg dose 30 minutes before treatment starts. If your chemotherapy is moderately emetogenic, take 8 mg 30 minutes before treatment, then another 8 mg dose 8 hours later. After chemotherapy, take 8 mg every 12 hours for 1 to 2 days.
- For radiotherapy, if you are receiving total body irradiation, take 8 mg 1 to 2 hours before each radiation session. If you are receiving a single high-dose fraction to the abdomen, take 8 mg 1 to 2 hours before treatment, then 8 mg every 8 hours for 1 to 2 days after. If you are receiving daily fractionated radiotherapy to the abdomen, take 8 mg 1 to 2 hours before each session, followed by 8 mg every 8 hours for each treatment day.
- For postoperative nausea and vomiting, take 16 mg one hour before anesthesia.
- For children aged 12 to 17 receiving moderately emetogenic chemotherapy, take 8 mg 30 minutes before treatment, then another 8 mg dose 8 hours later. After chemotherapy, take 8 mg every 12 hours for 1 to 2 days.
- For children aged 4 to 11, take 4 mg 30 minutes before chemotherapy, followed by 4 mg doses at 4 and 8 hours after the first dose. After chemotherapy, take 4 mg three times a day for 1 to 2 days.
- If you have severe liver disease with a Child-Pugh score of 10 or higher, do not take more than 8 mg per day.
Uncommon Side Effects: Heart problems, including chest pain, chest discomfort, high blood pressure, irregular heart rhythm, shortness of breath, or fainting.
Rare Side Effects: In case you experience blurred vision, consult your healthcare provider.

Ondansetron can cause changes in the heart’s electrical activity, including QT prolongation. Cases of Torsade de Pointes have been reported. Patients with heart conditions or electrolyte imbalances should be monitored with an ECG. Avoid ondansetron if you have congenital long QT syndrome.
Serotonin syndrome may occur, especially when ondansetron is used with serotonergic drugs. Symptoms include agitation, hallucinations, rapid heartbeat, sweating, tremors, and muscle stiffness. Severe cases may lead to seizures or coma. Stop ondansetron if symptoms appear and seek medical help immediately.
Ondansetron may mask symptoms of bowel blockage, especially in patients recovering from surgery or chemotherapy. Monitor bowel activity closely. Ondansetron does not promote intestinal movement and should not be used as a substitute for nasogastric suction.
Store Ondansetron HCl 4 mg at 20° to 25°C (68° to 77°F); excursions permitted to 15° to 30°C (59° to 86°F). Protect it from light.
