Indications: ProAmatine (midodrine hydrochloride) is indicated for the treatment of symptomatic orthostatic hypotension (OH). It is prescribed for patients whose lives are significantly impaired despite standard care. This includes support stockings, fluid expansion, and lifestyle changes. The medicine works by increasing 1-minute standing systolic blood pressure. This is considered a sign of potential clinical benefit. However, its effect on improving daily activities has not been proven. ProAmatine can cause a significant rise in supine blood pressure (systolic BP >200 mmHg). Because of this risk, it should be used with caution. Clinical trials are ongoing to confirm its benefits. After starting treatment, it should only be continued if patients experience significant symptomatic improvement. 

Do not use ProAmatine if you have severe organic heart disease, acute renal disease, urinary retention, pheochromocytoma, or thyrotoxicosis. Avoid this medication if you have persistent and excessive supine hypertension. 

Ingredients: ProAmatine contains midodrine hydrochloride as the active ingredient in strengths of 2.5 mg, 5 mg, and 10 mg. The inactive ingredients include colloidal silicone dioxide NF, corn starch NF, FD&C Blue No. 2 Lake (10 mg tablets), FD&C Yellow No. 6 Lake (5 mg tablets), magnesium stearate NF, microcrystalline cellulose NF, and talc USP. 

Directions: Follow your doctor’s prescription exactly.  

  • The usual dose of ProAmatine is 10 mg, taken three times a day. Take it during the daytime when you are active. 
  •  A typical schedule is in the morning, at midday, and in the late afternoon. Do not take it after 6 p.m. If needed, you can take doses every three hours, but not more often. 
  •  Single doses of up to 20 mg have been used, but they can cause severe and lasting high blood pressure while lying down. This happens in about 45% of patients at this dose. 
  •  Do not take ProAmatine after dinner or within four hours of bedtime. This helps prevent high blood pressure while you sleep. 
  •  Some people have taken more than 30 mg daily, but the safety and benefits of this have not been fully studied. 
  •  You should continue taking ProAmatine only if it improves your symptoms. Check your blood pressure regularly, both lying down and standing. Stop taking it if your blood pressure gets too high while lying down. 
  •  If you have kidney problems, start with 2.5 mg. This has not been fully studied, so take it with caution. 
  •  ProAmatine has not been studied enough in children. If you are 65 or older, you do not need a dose change. Men and women also do not need different doses.

Side Effects: Common side effects include Supine and sitting hypertension, paresthesia, pruritus (mainly of the scalp), goosebumps, chills, urinary urge, urinary retention, urinary frequency. 

Less common side effects include headache, feeling of pressure or fullness in the head, flushing, confusion, dry mouth, nervousness, anxiety, rash. 

Rare side effects include visual field defect, dizziness, skin hyperesthesia, insomnia, drowsiness, erythema multiforme, canker sores, dry skin, painful urination, difficulty urinating, weakness, backache, heartburn, nausea, stomach discomfort, gas, leg cramps.

Before you take ProAmatine, tell your healthcare provider if you have

  • a history of supine or sitting hypertension.
  • taken other medications that cause vasoconstriction, such as phenylephrine, ephedrine, dihydroergotamine, phenylpropanolamine, or pseudoephedrine.
  • heart problems or use cardiac glycosides (such as digitalis), beta-blockers, or other medications that affect heart rate.
  • urinary retention issues, as this medication affects bladder function.
  • diabetes and orthostatic hypotension.
  • a history of visual problems, especially if taking fludrocortisone acetate.
  • kidney problems, as this medication is eliminated through the kidneys.
  • liver problems, as the liver helps metabolize this medication.

ProAmatine may cause supine hypertension. You should not take it if you will be lying down for a long time. Keep your head elevated while sleeping. Report any symptoms like pounding in the ears, headache, or blurred vision. Stop the medication and talk to your doctor if these symptoms continue. Your blood pressure should be monitored if you take ProAmatine with vasoconstrictors. A slight slowing of your heart rate may occur. If you experience dizziness, fainting, or a slow pulse, stop using ProAmatine and consult your doctor.

Use this medication with caution if you have kidney or liver problems. Your doctor may need to adjust your dose. Your kidney function should be checked before starting ProAmatine. Certain over-the-counter cold medicines and diet aids can raise your blood pressure. Talk to your doctor before using them. ProAmatine should only be used in pregnancy if the benefits outweigh the risks. Animal studies have shown effects on fetal development. It is unknown if ProAmatine passes into breast milk. If you are nursing, use this medication with caution. This medication has not been studied in children. Its safety and effectiveness in pediatric patients are unknown.

Drug Interactions:

  • Combining midodrine with MAOIs like isocarboxazid, phenelzine, and tranylcypromine can lead to severe hypertension, making this combination unsafe. 
  • Drugs such as dihydroergotamine should be avoided with midodrine, as they can significantly increase blood pressure. 
  • Midodrine taken with digoxin may heighten the risk of bradycardia, atrioventricular (A.V.) block, or arrhythmias. 
  • Medications like doxazosin and prazosin can counteract midodrine’s effects, reducing its ability to manage orthostatic hypotension. 
  • Pseudoephedrine and phenylephrine may increase blood pressure when combined with midodrine, requiring caution. 
  • Levothyroxine can amplify midodrine’s blood pressure-raising effects, making careful monitoring necessary. 
  • Metoprolol may have a moderate interaction with midodrine, though its clinical significance varies. 
  • Cimetidine, Ranitidine, Metformin, Flecainide, Procainamide, Quinidine and Triamterene may interact with midodrine by affecting its renal clearance. 
  • Linezolid can enhance midodrine’s effects, potentially leading to excessive blood pressure elevation. 

Store at 25°C (77°F), with permitted excursions between 15°C and 30°C (59°F to 86°F). Keep out of reach of children.
