Otezla (apremilast)

Indications

  • Psoriatic Arthritis: Otezla is used to treat active psoriatic arthritis, helping to reduce pain, swelling, and joint damage.
  • Plaque Psoriasis: It is prescribed for moderate to severe plaque psoriasis, particularly for patients who are candidates for phototherapy or systemic therapy.
  • Oral Ulcers Associated with Behçet’s Disease: Otezla is effective in reducing the frequency and severity of oral ulcers in adults with Behçet’s Disease.

Dosage and Administration

  • Take Otezla exactly as prescribed by your doctor.
  • Swallow the tablet whole with a full glass of water.
  • Take the medication at the same time each day, with or without food. If you miss a dose, take it as soon as you remember unless it is almost time for your next dose.
  • Gradually increase the dosage as directed by your doctor during the initial treatment phase to minimize gastrointestinal side effects.
  • Do not take more than the recommended dose.
  • Avoid skipping doses or stopping the medication without consulting your doctor.
  • Do not crush, chew, or break the tablet; it should be swallowed whole.
  • Avoid taking Otezla if you have a known allergy to apremilast or any other ingredients in the tablet.

Mechanism of Action

Otezla is a phosphodiesterase 4 (PDE4) inhibitor. It works by reducing the activity of PDE4, an enzyme that breaks down cyclic AMP. By inhibiting PDE4, Otezla helps to regulate inflammation within cells, reducing the symptoms of inflammatory conditions like psoriatic arthritis and plaque psoriasis.

Contraindications

  • Do not use Otezla if you are allergic to apremilast or any other components of the tablet.
  • Avoid use if you have severe renal impairment unless specifically directed by your doctor.
  • Not recommended for use in pregnant women, as the effects on the unborn baby are not well studied.
  • Avoid use if you have a history of depression or suicidal thoughts without discussing it with your doctor.
